    PPP are seeking to recruit a Care Co-ordinator (21,450 pro rata or 11.45 per hour) to support their Care at Home and housing support staff. This is a new position and will work alongside our existing Care Co-ordinator position to provide 7 days a week back up and support to our growing care team.

    The candidate will work on a daily basis with the Corporate Administration team, Registered Manager and the Care Co-ordinator and care staff to ensure we are running the service to a high standard and meeting the needs of our 79 service users.

    They will be responsible for meeting new service users and their family, support plans, risk assessments, ordering medication, record keeping, service users rotas, day to day changes with NHS staff, family members and our service users. They will work in partnership with NHS and social work to undertake reviews, and update support plans.

    They will also provide cover on shift when required to cover short term sick, or occasional holiday cover. This allows them the opportunity to work alongside care staff, and meet the family and service users in their own environment.

    They will support staff every second weekend, that will be a mix of occasional weekend shifts, providing on call cover and dealing with any urgent issues and keeping staff updated of the changes.

    They will be responsible for all aspects of Data Protection (2018)PPP Application Form with Equality and Diversity Monitoring.pdf relating to the care at home service. The ability to implement our ongoing Quality Improvement Plan to ensure we keep our very high care inspectorate grades.

    Required:

    They will have a background in the care sector and be registered as a senior with the SSSC and hold the recognised qualifications required for registration.

    Successful candidate must also apply for PVG scheme membership.

    Excellent computer skills, confident with excel, word, emailing and office processes.

    They will be experienced at report writing, record keeping, quality assurance and health and safety, duty of candour, processes.

    Familiar with the Health and Social Care Standards, and SSSC Codes of Practice.

    They will be professional at all times, compassionate and able to relate and emphasis with families during difficult circumstances, able to work under pressure, and provide clear communication and support to care staff, the wider team, families and the partners we work alongside.

    Promote the organisation, and the services we provide.

    They will have excellent communication skills, and be able to work under pressure, in a workplace where service user’s circumstances can be very fluid, and we need to be able to meet their requirements and support them at very short notice. The job will be rewarding, but also demanding and challenging and will require being on call to support staff on occasions in the evenings of your work pattern.

    Daily access to a car with business insurance is necessary.
