If you write a letter to T-Mobile at the following address they will send you your unlock code free of charge. Just got mine today.

Include the manufacturer, model and phone IMEI code. You get the code by pressing *#06# on your phone. Supply a return address and email address. They will email the code to you with instructions.
It might only be for ex customers but not sure.
